Columbus House opened its doors in 1982 to provide services for men and women at least 18 years of age. Our goals quickly broadened from the mere provision of food and shelter and simple survival, to understanding and working toward overcoming the problems which cause people to become homeless.
Columbus House, along with its core of loyal supporters, friends, and volunteers, has remained committed to fulfilling its mission : "To serve people experiencing homelessness or at imminent risk by providing life-saving outreach, shelter and housing and by fostering their personal growth and independence. We advocate for and create affordable housing to end homelessness in our communities."
Purpose
Under the direction of the Program Manager of Case Management Services, the Outreach Case Manager seeks to outreach and engage men and women over the age of eighteen who are homeless and struggle with unresolved issues of mental illness and substance abuse and are not receiving services and to link them to agencies and activities that will stabilize their lives, support their sobriety, and empower them to end their homelessness.
Duties and Responsibilities
- Conduct street outreach
- Provide linkages for clients who are in need of social, mental health, educational, vocational, recreational, spiritual, and or substance abuse services
- Provide transportation to appointments for clients
- Coordinate and collaborate with other agencies and social service providers
- Engage in “street” outreach in all kinds of weather conditions
- Attend weekly and monthly agency, program and network meetings
- Monitor all cases assigned and complete all documentation / charting required by Columbus House and its relevant funders
- Complete First Contact Forms
- Maintain and update weekly charting and movement reports
- Enter and maintain all client charts accurately and timely into the Homeless Management Information System database Case Worthy
- Other related duties as assigned in support of a 17 client case load
Qualifications
Supervised by : Program Manager of Case Management Services
Salary : $25.50-$27.50 per hour based upon education and experience plus excellent benefits
Schedule : Sunday 10am- 6pm, Monday 9am-5pm, Tuesday 10am- 6pm, Wednesday 9am- 5pm
