What are the responsibilities and job description for the Diabetes Educator - LCHC Education position at Comanche County Memorial Hospital?
DEFINITION :
The Diabetes Educator provides diabetes self-management education in an interactive, collaborative, ongoing process involving the person with diabetes. The process includes assessing education needs, identifying goals, intervention to achieve goals, evaluating attainment of goals, as well as documentation. Maintains standards of professional performance and development, i.e., ethics, collaboration, research and uses resources effectively and efficiently.
EDUCATION :
As required by Commission on Dietetic Registration.
REGULATORY REQUIREMENTS (IF APPLICABLE) :
Licensed Dietitian in Oklahoma.
MINIMUM QUALIFICATIONS :
If not CDE, 8-10 hour Diabetes Training Program. Must be obtained within a year.
PREFERRED QUALIFICATIONS :
Certified Diabetes Educator.
1-3 years in institutional clinical setting of similar nature and size or in / out patient diabetes center.
