Job Overview: 
Position: Case Manager, RN
Schedule: Full-time, Monday to Friday, and/or on-call
Hourly range:  $40-$55/hour. DOE
Reports to:  Executive Director of Patient Care
Medical specialty:  Hospice and Palliative Care
Physical setting: Patient private homes and Long-Term Care Communities

This role offers a fulfilling opportunity for a hospice Case Manager to provide compassionate care in a supportive environment, free from corporate bureaucracy and excessive overtime. Responsibilities:

  1. Patient Care and Assessment: Monitor patients' conditions, assess their needs, and provide the best possible care.
  2. Symptom Management: Observe and interpret patients' symptoms, communicating to the Executive Director of Patient Care and updating the EMR system.
  3. Collaboration: Work alongside the entire Interdisciplinary team creating a positive experience of comfort to the patient and family.
  4. Procedures and Documentation: Perform routine procedures (e.g., blood pressure measurements, administering injections) and maintain accurate patient records.
  5. Medication Management: Monitor patients' medication needs and convey insights to the Medical Director and Executive Director of Patient Care.
  6. Supportive Environment: Foster a supportive and compassionate environment for patients and their families.
  7. Professional Development: Expand knowledge by attending educational workshops, conferences, and online training.
Qualifications:

  • Experience: Proven experience as a registered nurse;  one year hospice experience is preferred.
  • Education:  RN degree from an accredited nursing program.
  • License: Active and unencumbered Oregon nursing license.
Skills
  •  Excellent knowledge of hospice nursing care methods and procedures.
  •  In-depth knowledge of health and safety guidelines.
  •  Outstanding communication, interpersonal, organizational, and multi-tasking skills.
  •  Curious, Compassionate, and Resilient with excellent problem-solving skills towards comfort measures.
