We are seeking a compassionate and dedicated Home Care Nurse to provide high-quality care to patients in their homes. Responsibilities
- Deliver personalized nursing care to patients in their homes, ensuring comfort and safety.
- Monitor patient health by observing vital signs and assessing overall well-being.
- Develop and implement individualized care plans based on patient needs and preferences.
- Provide support for patients with dementia, memory issues, and other cognitive challenges.
- Assist with daily living activities, including personal hygiene, meal preparation, and mobility assistance.
- Collaborate with family members and healthcare teams to ensure comprehensive care.
- Maintain accurate records of patient progress and communicate any changes to the healthcare team.
- Administer medications and treatments as prescribed by physicians.
- Provide education and support to patients and families regarding health management.
Experience
- Proven experience as a Home Care Nurse or similar role in a healthcare setting.
- Strong skills in social work, patient observation, and monitoring.
- Experience working with individuals with developmental disabilities or autism is preferred.
- Familiarity with CPR techniques and emergency response protocols.
- Knowledge of senior care practices and the unique needs of elderly patients.
- Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare professionals.
