Responsibilities
- Deliver comprehensive nursing care to patients in their homes, ensuring adherence to care plans.
- Monitor and assess patient health status, including vital signs and overall well-being.
- Administer medications and treatments as prescribed by physicians.
- Provide support for activities of daily living (ADLs), including personal hygiene, mobility assistance, and meal preparation.
- Educate patients and their families about health conditions, treatment plans, and preventive care.
- Maintain accurate and timely documentation of patient progress and care provided.
- Collaborate with interdisciplinary teams to ensure coordinated care for patients.
- Uphold resident rights and dignity while providing compassionate care.
- Assist with dementia care strategies to support cognitive function and emotional well-being.
Requirements
- Experience in home health care or similar settings is preferred.
- Knowledge of CPR techniques and certification is required.
- Familiarity with patient monitoring equipment and procedures.
- Understanding of resident rights within home community care settings.
- Experience in hospice care or group home environments is a plus.
- Strong interpersonal skills with the ability to build rapport with patients and families.
- Ability to work independently while managing multiple priorities effectively.
