Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, ensuring quality outcomes, and facilitating communication among healthcare providers. This role is crucial in managing the continuum of care for patients, particularly those with complex medical needs. The Registered Nurse Case Manager will work collaboratively within a multidisciplinary team to provide exceptional patient-centered care.
Duties
- Conduct comprehensive assessments of patients' medical histories and current health status.
- Develop and implement individualized care plans based on diagnostic evaluations and patient needs.
- Coordinate care transitions between various departments, including ICU, ER, and outpatient settings.
- Serve as a liaison between patients, families, and healthcare providers to ensure effective communication.
- Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
- Educate patients and families about their conditions, treatment options, and available resources.
- Maintain accurate documentation of patient interactions and care plans in compliance with hospital policies.
- Participate in quality improvement initiatives to enhance patient care delivery.
Experience
- Current Registered Nurse (RN) license in the state of practice.
- Experience in a Level I Trauma Center or similar high-acuity environment preferred.
- Background in critical care nursing (ICU, ER, PICU) is highly desirable.
- Familiarity with medical terminology and hospital medicine practices.
- Proven ability to perform diagnostic evaluations and manage complex cases effectively.
- Strong interpersonal skills with the ability to work collaboratively in a team setting.
- Previous experience in case management or care coordination is an advantage.

Pay: $75,554.00 - $79,350.00 per year
