Job Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring that patients receive the necessary services and support throughout their treatment journey. You will work closely with patients, families, and healthcare providers to develop individualized care plans that address medical, emotional, and social needs. Your expertise in behavioral health, medical terminology, and various clinical settings will be essential in delivering high-quality care to our patients.
Responsibilities
- Assess patient needs through comprehensive diagnostic evaluations and develop tailored care plans.
- Coordinate and manage patient care across various settings including hospital, ICU, and inpatient facilities.
- Collaborate with multidisciplinary teams to ensure effective communication and continuity of care.
- Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
- Educate patients and families about treatment options, resources available, and self-care strategies.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
- Utilize knowledge of pediatrics, acute care, and physiology to provide specialized support as needed.
- Document all interactions with patients accurately in accordance with medical terminology standards.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in behavioral health, pediatrics, ICU, or acute care settings preferred.
- Strong understanding of medical terminology and diagnostic evaluation processes.
- Excellent communication skills with the ability to engage effectively with patients and families.
- Ability to work collaboratively within a multidisciplinary team environment.
