- Monitor and document changes in client behavior or health status, reporting any concerns to the nursing staff.
- Assist clients with mobility and transfers when necessary.
- Engage clients in social activities to promote mental stimulation and well-being.
Skills
- Experience in nursing homes, assisted living, or long-term care settings is preferred.
- Strong ability for patient observation and understanding of long-term care needs.
- Proficient in providing home health services with a focus on client dignity and respect.
- Dementia training certificate is required.
- Excellent communication skills to effectively interact with clients, families, and healthcare teams.
