Comfort Home Care is in search of a full-time weekend RN or LPN in Lowell and the surrounding areas. Our nurses visit a diverse population, mostly with mental health diagnoses, performing nursing assessments, communicating with other providers, and maintaining patients in the community as independently as possible.

Day to day responsibilities for a Comfort Home Care Nurse :
- Provide initial and ongoing assessments to identify the client's medical needs, level of functioning and ability to communicate and learn.
- Initiate the plan of care, necessary revisions, and perform OASIS assessments at appropriate time points when applicable.
- Initiate and perform appropriate nursing interventions.
- Coordinate services and inform the physician and other personnel of changes in the client's condition and needs.
- Collaborate with patient / family and other health care providers and / or community resources with planning of care and discharge.
- Prepare clinical and progress notes and submit all required paperwork in a timely manner according to agency policy and procedure.
