Position Summary:
The Client Care Manager is responsible for managing the client experience from their first visit through the continuity of their care. The role of the Client Care Manager begins in the office, where you will assist in developing and monitoring ongoing caregiver and client relationships through the proactive management of the client schedule. This will include working with the scheduler where you will select and apportion work among the caregiver staff based on skills, performance, and the overall needs of the client.
The Client Care Manager will facilitate communication between the caregiver and the client, ensuring that all client needs are met through the delivery of quality care, as well as identifying and resolving potential client issues that may arise. Finally, the Client Care Manager will work with our office staff to ensure that we have adequate staffing levels and availability to service potential clients.
Homecare Experience Required
Essential Functions:
A Client Care Manager:
- Answers potential client inquiries whether made via phone or web and sells and qualifies them for home visits.
- Selects and apportions work among caregivers based on skills, performance, and overtime requirements.
- Proactively manages the ongoing internal client relationship for existing clients. Uses independent judgement to accomplish problem resolution, increasing support for clients, and quality assurance. Anticipates needs and advises families.
- Responsible for regular and routine assessments of Clients including identifying and making recommendations regarding any safety concerns of caregivers or clients.
- Coordinates care with Caregivers, family members, and other care providers such as Hospice.
- Develops relationships with referral sources and participates in “Marketing
- events.
- Follows all office communication and documentation protocols to ensure excellent service to clients, staff, potential and inactive clients.
- Participates in the on-call rotation by carrying the agency’s emergency cellular phone on a rotating basis. While on-call, handles all client, caregiver, and scheduling issues that occur and conducts all new client intakes as needed.
- May be required to work Caregiver shifts in case of emergencies.
- Participates in formal and impromptu management meetings and in the development of systems to improve care coordination, client satisfaction, employee performance and retention and office operations.
- Assists the other members of the management team as needed. Completes special projects as assigned.The Client Care Manager will work closely with internal and external contacts including Managers, Client Care Managers, HR, Scheduling, Caregivers, Clients and Family Members, Referral Sources, and outside Vendors.
