Are you a compassionate and skilled Registered Nurse with a passion for leading teams and improving lives? At Comfort Keepers, we’re seeking a Case Manager Supervisor (RN) to help ensure the continuity and quality of in-home care for our valued clients, including those receiving Medicaid services.
About the Role
As the Case Manager Supervisor, you’ll lead and support a team of case managers and caregivers. You'll ensure clients receive exceptional care aligned with our high standards. This role plays a critical part in evaluating new clients, supervising ongoing care, and maintaining strong communication with healthcare partners and family members.
Key Responsibilities
Conduct initial health assessments and determine care needs after client referral.
Create and oversee care plans tailored to each client.
Supervise caregivers and provide ongoing evaluations and support.
Serve as the liaison between clients, families, caregivers, Medicaid, and healthcare providers.
Monitor service satisfaction and make necessary adjustments.
Manage caregiver on-boarding and training.
Participate in on-call rotation and assist in direct care when needed.
Who You Are
Licensed RN in good standing with a minimum of 2 years of relevant experience .
Excellent communicator and compassionate leader.
Experienced in case management, client assessment, and Medicaid processes (preferred).
Comfortable in both office and field work , including home visits.
Able to manage multiple priorities with attention to detail and efficiency.
Strong understanding of healthcare compliance and best practices.
What You’ll Need
Active RN license
Proficiency with computers and healthcare systems
Reliable transportation and ability to travel locally
Experience managing teams is a plus
