Job description
The Lead DSP is responsible for providing leadership and supervision to the DSP’s, troubleshoot staffing issues, scheduling, covering open shifts/call offs and performing routine compliance checks of their ISL and client records. Lead DSP’s will be required to cover the first 8 hours of overtime prior to asking Residential Support Professional (RSP) to cover the shift.
This position requires availability 24 hours a day 7 days a week and ability to work mandatory overtime, and the ability to provide support to their team, perform home visits, and respond to emergencies and phone calls that will partially occur on evenings, overnights, weekends, and holidays. COHH Leads are expected to be able to report to work due to call ins and emergency situations due to this, COHH has to be a priority.
The individual requires female staff per her care plan, and is considered to be behavioral. Individuals with experience working with ASD will be considered first.
Compensation and Reporting
LDSP is an hourly position working a 36-40 hour work week and directly reports to their Residential Support Professional & Comfort of Home Healthcare Administration.
Work week is from Saturday through Friday.
Position requires working flexible hours and needs to report schedule to management monthly.
Supervisory Responsibilities
The LDSP is a team leader/coaching role in Comfort of Home Healthcare and is responsible for supervision only. Any disciplinary actions are the sole responsibility of Comfort of Home Healthcare Administration.
Staffing Responsibilities
LDSP is responsible for ensuring that Comfort of Home Healthcare has adequate staffing in the home they are responsible for.
· LDSP is to be available to Comfort of Home Healthcare clients and Comfort of Home Healthcare office/administration 24/7. (When time off is needed, coordinate it with Administration so that all employees are aware and will not call at this time)
· LDSP will take the lead in all aspects of scheduling care for their home.
· LDSP will take the lead in filling open shifts and call-ins for their house and help other Team Leaders filling shifts with their homes.
· LDSP may be called upon to cover shifts him/herself when Comfort of Home Healthcare is short on replacement staff. Team Leaders can then adjust their schedules in the next couple days/week to accommodate the extra hours of work.
· LDSP will make a commitment for being the hardest worker in the house they are responsible for. LDSPs are to lead by example by making their shifts and being punctual.
· LDSP is solely responsible for the operation of their respective home.
Quality Control Home Visits
Monthly home visits are mandatory to insure the quality of Comfort of Home Healthcare services and is one of the most important functions of this position.
· Monthly home visits will consist of checking on all documentation so that it is being done correctly and to assess any staffing issues.
· Monthly home visits need to be coordinated with the Residential Support Professional (when applicable) to ensure there is no redundancy in home visits.
· Accurate documentation will be required for all home visits.
· Record all home visits on home visit documentation log and turn them into the office monthly.
LDSP Responsibilities
It is the responsibility of the Lead Direct Support Professional (LDSP) to ensure that the house is run smoothly. More than 44 hour work weeks are expected initially until the home is adequately staffed with quality caregivers. It is the responsibility of the LDSP to maintain quality, professional relationships with the family and their staff and any other agency involved such as Hospice or Home Health. Professionalism is highly important in this role. LDSPs are a direct reflection on the agency.
· LDSPs are to log all activities in their daily activities and report to office monthly.
· LDSPs are solely responsible that all documentation is accurate and complete in their homes. This documentation is to be checked daily.
· LDSPs are responsible for strictly adhering/coaching to any Care Plans (DMH) or Plan of Care (DHSS) by themselves and all staff in the home they are responsible for.
. Documentation must be kept and updated daily to keep outings and activities in accordance to outcomes in these care plans.
. Money/ travel logs must be 100% accurate 100% of the time.
· Mangers are responsible for reporting anything out of the ordinary to the office. Examples are:
o Client being injured
o Med Errors/ Event Reports
o All staffing issues (tardy/call-ins)
LDSP Job Performance Assessments
All LDSPs will initially be placed as a trial basis for 3 months (90 days). After this period, Residential Support Professionals will meet with Comfort of Home Healthcare Administration and evaluate their job performance. If kept on as the permanent LDSP, annual job performance evaluations will be conducted thereafter. If management/ office does not feel performance is acceptable, either an additional evaluation period will be issued or a determination that the job is too difficult and be placed back as a direct care staff.

All LDSPs are privy to confidential information and its business practices. All of Comfort of Home Healthcare, LLC’s activities are to be treated as confidential. All client and employee information are to be kept confidential even if relieved of their position with Comfort of Home Healthcare. Contact with Comfort of Home Healthcare’s employees and clients if not working for Comfort of Home Healthcare is strictly forbidden.
LDSP Accountabilities Summary
· Weekly/Monthly house checks on all homes on all shifts
· Audit and maintain accurate participants documentation to ensure they are properly completed in accordance with COHH policy and MMAC compliance.
· Troubleshoot and assist staffing call in’s and scheduling issues.
· Work call in shifts when replacements can not be found.
· Guardian updates & contacts
· Monthly reviews submitted to Residential Support Professionals (RSP)
· Complete ISL audits monthly
· Train all new Direct Support Professionals (DSP).
· Provide ongoing staff trainings specific to their home.
· Attend monthly LDSP meetings
· Attend/plan meetings, service monitoring’s & Doctors Appointments
· Participate in pre-& post transition meetings
· Attend required trainings & webinars
· Facilitate new hire and ongoing DSP trainings in the home
· EMT review & submission
· Spending ledger entry
· Schedule house meetings
· Calendar tracking/planner to keep track of all participants appointments
· Schedules need completed in Rosemark for the following month by 23rd
· Other duties as assigned by Administration.
· Submit internal audits of homes to RSPs every month.
· Develop a daily agenda/task planner; submit weekly agenda to RSP.
· Communicate with Coordinators weekly updates/changes
· Create, maintain, and submit internal home calendar with weekly/monthly schedule.
· Maintain accurate EVV time sheets for the home.
Certifications: (If you have these certifications that is a plus but we will set up training for you if you do not have them) First Aid & CPR, Level 1 Med-Aide Or DDMA, Abuse and Neglect, PBS, EOE/D/V
All employees must pass background check and commit to drug screenings. Must have valid driver’s license, reliable car, proof of auto insurance, ability to follow instructions, and have excellent time management and multitasking skills.
