Position Overview:
The Client Care Manager will oversee the daily care operations of clients, ensuring their needs are met with the utmost professionalism and compassion. This role will include monitoring and coordinating care plans, conducting client assessments, maintaining caregiver schedules, and ensuring compliance with company standards. Additionally, the Client Care Manager will step in to cover shifts when caregiver availability is limited, ensuring continuous, high-quality care for clients.
Key Responsibilities:
- Client Care Coordination:
- Develop, implement, and maintain individualized care plans for clients, ensuring their needs are met and updated as necessary.
- Conduct initial and ongoing client assessments to determine care requirements and goals.
- Monitor and evaluate caregiver performance, ensuring quality and compliance with care plans.
- Regularly check in with clients and families to ensure satisfaction and address any concerns or adjustments needed.
- Caregiver Support & Scheduling:
- Manage caregiver schedules to ensure clients receive uninterrupted care.
- Train and orient new caregivers, providing ongoing support and performance feedback.
- Address and resolve scheduling conflicts or caregiver absences by filling in as needed when caregiver coverage is unavailable.
- Foster a positive, collaborative work environment for caregivers.
- Client & Family Communication:
- Serve as the primary point of contact for clients and their families, addressing any issues or concerns promptly.
- Keep clients and families informed of any changes or updates to the care plan or schedule.
- Provide emotional support and guidance to clients and families navigating the care process.
- Compliance & Reporting:
- Ensure that all services comply with local, state, and federal regulations and company policies.
- Maintain accurate, up-to-date client records and documentation.
- Participate in regular audits to ensure care delivery standards are met.
- Shift Coverage:
- Step in to cover shifts when caregivers are unavailable due to absence, illness, or scheduling gaps.
- Provide hands-on caregiving support, including personal care, mobility assistance, and companionship as needed.
- Ensure smooth transitions between caregivers and maintain continuity of care.
Qualifications:
- Certified Nursing Assistant (CNA) preferred.
- Proven experience in home care, caregiving, or a related healthcare field.
- Strong communication and interpersonal skills with the ability to build rapport with clients, families, and caregivers.
- Excellent organizational and problem-solving skills, with the ability to manage multiple tasks and priorities.
- Ability to work independently and as part of a team in a fast-paced environment.
- Compassionate, reliable, and dedicated to providing exceptional care to clients.
- Valid driver’s license and reliable transportation.
Physical Requirements:
- Ability to perform physical tasks such as lifting, transferring, and assisting clients with mobility.
- Comfortable with a flexible work schedule, including covering shifts on weekends or evenings as needed.
