We are seeking a dedicated and experienced Social Work Supervisor - Per diem, to join our team in providing exceptional care and support to our clients. This role requires a thorough knowledge of home healthcare, medical records, case management for ComfortPlus Home Healthcare.
Duties
- Oversee the development and implementation of individualized care plans for clients, ensuring they meet their unique needs.
- Conduct regular assessments of client progress and adjust care plans as necessary.
- Maintain accurate medical records in compliance with HIPAA regulations, ensuring confidentiality and security of patient information.
- Collaborate with interdisciplinary teams to provide comprehensive support for patients, including mental health counseling and home community care services.
- Provide training on medical terminology and clinical counseling techniques to enhance team skills.
- Stay updated on trends in social work practice
Experience
- A Master's degree in Social Work (MSW) from an accredited institution is preferred.
- A minimum of three years of experience in social work.
- Strong knowledge of case management practices, medical records management, and mental health counseling techniques is essential.
- Familiarity with HIPAA regulations and ethical standards in social work practice is crucial.
- Experience working with diverse populations is highly desirable.
- Excellent communication skills, both verbal and written, are necessary for effective collaboration with clients and team members alike.
