Role Description
The Continuity of Care Specialist is a key member of our team, responsible for ensuring the highest level of service delivery to our clients. This role requires a strong understanding of human services principles and practices, as well as excellent communication and interpersonal skills.
Duties and Responsibilities:
- Acquisition and coordination of all services required by residents to attain treatment goals.
- Supervision of resident fund management, ensuring entitlements are in place and not jeopardized by under spending or mismanagement.
- Active engagement in resident advocacy with internal and external programs and agencies.
- Assistance with program and facility compliance with internal and external policies, statutes, and regulations.
Essential Qualifications:
- Bachelor's degree in Human Services or related field required.
- Minimum one year of experience working in human services, with a focus on quality improvement.
- Valid PA State Criminal, FBI, and Child Abuse clearances.
- Valid driver's license and reliable transportation.
