About the Role
The Continuity of Care Specialist will play a vital role in our organization's mission to provide high-quality services to individuals in need. This position requires a strong understanding of human services principles and practices, as well as excellent communication and interpersonal skills.
Key Duties and Responsibilities:
- Acquisition and coordination of all services required by residents to attain treatment goals.
- Supervision of resident fund management, ensuring entitlements are in place and not jeopardized by under spending or mismanagement.
- Active engagement in resident advocacy with internal and external programs and agencies.
- Assistance with program and facility compliance with internal and external policies, statutes, and regulations.
Qualifications:
- Bachelor's degree in Human Services or related field required.
- Minimum one year of experience working in human services, with a focus on quality improvement.
- Valid PA State Criminal, FBI, and Child Abuse clearances.
- Valid driver's license and reliable transportation.
