Tioga Community Care Center is seeking an MDS Clinical Care Coordinator!
Available shift: Monday-Friday 8a-4p, Call Rotation

Essential Responsibilities:
- Provides each resident with professional nursing care, assessments and evaluations.
- Responsible for the management of nursing activities performed by Neighborhood/Household (NH/HH) staff ensuring that the highest degree of quality care is maintained at all times
- Works with a team of cross-trained staff to coordinate the functions of the NH/HH
- Has 24 hour responsibility for clinical functions of the NH/HH
Qualifications
- A current, valid Louisiana registered nurse (RN) license is required
- Minimum of one (1) year experience in long term care, prior MDS experience is preferred.
- Ability to effectively communicate, direct and delegate tasks.
Other Qualifications
- Has not been excluded from participating in Medicare and Medicaid programs.
- Maintains eligibility to participate in Medicare and Medicaid programs throughout employment.
- Participate in pre-employment and annual Tuberculosis screening.
