Position Title: Rehabilitation Direct Care – Psychiatric Rehabilitation Program (PRP)
General Description: Committed to Change provides services to children and adults in order to help manage their symptoms of mental illness. The PRP direct care worker will work with individuals directly, assist them with developing their Rehabilitation plans, links clients to needed community resources, social benefits and skills training. Our PRP direct care workers work with the individuals we serve, in-home, in-office and within the community to provide them with individualized care.
Must work well on a team, know basic computer skills, possess good verbal and written communication skills and have a valid driver’s license and your own vehicle.
Summary of Essential Functions, Responsibilities and Duties :
Assist individuals develop their Individual Rehabilitation Plans and complete the written plan
Write monthly progress notes
Participate in monthly team meetings
Communicate with the Rehabilitation Specialist weekly to ensure continuum of service
Provide program coverage in all areas and in all aspects, including on site services and documentation
Provide off site services, including in the home
Other duties as assigned
Essential Physical Requirements Include:
Driving personal vehicle. You will NOT be required to transport patients.
This job will begin as part-time, fee-for-service position at $20 per client contact and transition to a 40hr/week hourly position as your caseload grows.
This is currently a hybrid position, with some hours able to be worked from home.
