Common Threads seeks a Healthcare Partnership Manager to join our Programs team and advance our mission to champion improved community health through the power of food and nutrition.
The Healthcare Partnership Manager will lead the development and implementation of the SNAP-Ed Fruit and Vegetable Prescription (FVRx) Program as part of Common Threads’ SNAP NY grant initiatives. This role will focus on forging partnerships with healthcare organizations, training providers, and connecting underserved families to nutrition education and resources. In New York, the SNAP-Ed Fruit and Vegetable Prescription (FVRx) Program will provide participating patients with vouchers to purchase fresh produce and access to workshops, supporting healthier lifestyles and promoting food security. In Los Angeles, the Healthcare Partnership Manager will contribute to programming by developing and managing partnerships with School-Based Health Centers (“Wellness Centers”) and referral processes for increasing program utilization and Medicaid reimbursement.
This position is critical to bridging healthcare and nutrition education, addressing systemic barriers to healthy eating, and contributing to Common Threads’ efforts to improve dietary health, reduce food insecurity, and empower families.

- Location: New York City (NYC)
- Salary Range: $70,000- $72,500/year
- Start Date: March 1st, contingent on NY State contract signature.
Responsibilities:
- Develop and manage strategic partnerships with healthcare providers, SNAP outreach organizations, and local produce vendors to ensure seamless program delivery.
- In collaboration with program operations, develop referral processes and coordination with healthcare provider partners to support closed referral loops and Medicaid reimbursement pathways.
- Train healthcare providers on Common Threads’ referral processes and nutrition education offerings, equipping them to connect patients to FVRx programs.
- Coordinate and manage the implementation of weekly workshops for program participants, ensuring high-quality delivery of curriculum, food demonstrations, and produce distribution.
- Oversee voucher systems for participating patients to redeem fresh produce from local farmers’ markets and produce vendors, supporting the local economy.
- Act as a case manager, ensuring participants successfully access and benefit from Common Threads’ programs and services.
- Monitor and evaluate program impact, collecting data to report on outcomes and continuously improve programming.
- Collaborate with Common Threads’ evaluation team to integrate feedback and emerging research into program enhancements.
- Stay informed on best practices in Fruit and Vegetable Prescription (FVRx) programs and the Food as Medicine movement.

Qualifications
Required:
- Bachelor’s degree in public health, healthcare administration, nutrition, or a related field.
- 3 years of experience in public health, healthcare partnerships, or nutrition program implementation.
- Proven success in managing partnerships with healthcare organizations and community stakeholders.
- Strong project management and organizational skills, with an ability to meet grant deliverables.
- Experience in program evaluation and data collection to measure impact.
- Excellent communication skills, with the ability to engage diverse audiences, including healthcare professionals, community members, and local farmers.
- Knowledge of nutrition education and its connection to addressing diet-related illnesses.
- Proficient in Salesforce (preferred) or similar CRM software
- Proficiency in Microsoft Office and familiarity with data management tools.
Preferred:
- Master’s degree in public health or related field.
- Experience with Medicaid, SNAP-Ed programming or other federally funded nutrition initiatives
- Familiarity with Medicaid workflows or managed care systems.
- Commitment to advancing health equity and racial justice.
Work Environment
- Remote position with occasional travel required.
- Home office environment with standard office equipment.
- Some evening or weekend work may be required based on program needs.
Physical Demands
- Regularly required to talk, hear, stand, walk, and use hands to handle or feel materials.
- Occasionally required to lift and move supplies up to 25 pounds.
