We are seeking a skilled and experienced Registered Nurse (RN) to join our team as a Hospice Care Provider. As a member of our team, you will have the opportunity to work with patients and their families in a variety of settings, including patient homes and healthcare facilities.
Responsibilities
- Provide high-quality nursing care to patients in their homes
- Develop personalized care plans in collaboration with patients and their families
- Manage symptoms and pain, and provide medical treatment and support
- Work closely with other healthcare professionals to ensure comprehensive care
Requirements
- RN license in the state of Wisconsin
- Associate's degree minimum required
- 1 year of nursing experience preferred
