What are the responsibilities and job description for the STAR Program Cultural Community Liason position at Commonpoint Queens?
STATEMENT OF THE AGENCY :
Commonpoint (formerly the Samuel Field Y and the Central Queens Y) is a non-profit, multi service agency with flagship sites in Little Neck, Bayside, Forest Hills and 33 additional sites throughout the community that provides a diverse array of programs and services to over 35,000 mission is to sustain and enhance the quality of family and individual life for all members of our community, regardless of religion, ethnic or cultural background, national origin, gender identity or expression, sexual preference, age, or disability, within a pervasive and inclusive environment that facilitates individual growth, meets individual needs and builds community. DUTIES AND RESPONSIBILITIES :
- Responsible for providing direct services to trauma-affected newcomers.
- Has primary responsibility for cases assigned.
- Assists newly enrolled trauma-affected newcomers in acclimating within the organization, and is primarily responsible for guiding them through their work within the agency and with external partners.
- Documents all work with clients in the agency’s case management software system.
- Responsible for providing high quality care.
- Advocates for clients within the organization and externally when clients are referred to network providers.
- Will participate in the recruiting of clients for the Client Advisory Board.
- Ensures that services are delivered according to the STAR program’s policies and procedures and operational guidelines.
- Assists to maintain and continue to develop the STAR program’s services and form partnerships where appropriate.
- Compiles, maintains and reports on program statistics, as needed.
- When indicated, conducts home visits and engages in telehealth sessions with clients.
- Other duties as assigned.
