As a Project Manager, Integrations you'll play an integral role in managing all phases of integration projects, from pre-work planning and execution to go-live and post-implementation support to ensure successful completion of deliverables within the established time-line. Other responsibilities include:
- Manages all phases of integration projects, from pre-work planning and execution to go-live and post-implementation support, collaborating with the Director of Growth to ensure successful completion of deliverables on an established time-line.
- Collaborates closely with the Director of Growth to ensure comprehensive stakeholder engagement throughout all phases of integration projects, guaranteeing effective planning and task execution.
- Adheres to the Corporate Compliance Program, including HIPAA confidentiality standards for all protected health information.
- Leads the planning, scope, and implementation efforts for multiple projects or phases of larger projects.
- Manages project life-cycle aspects, including budget, resources, and time-lines, to achieve financial and quality goals.
- Collaborates with project sponsors, process owners, subject matter experts, and other stakeholders to develop implementation plans, establish performance metrics, and set improvement targets. Engages regularly with second- and third-tier project managers to ensure alignment with organizational strategy and priorities.
- Serves as the main point of contact for vendors involved in project delivery, managing vendor relationships, time-lines, and escalations to ensure objectives are met within scope, time, and budget requirements.
- Represents the department in cross-functional workgroups and committees. Proactively identifies and resolves issues, escalating to second- and third-tier project managers as necessary.
- Ensures projects adhere to organizational standards and project management best practices, managing scope, schedule, and budget constraints while applying risk management strategies to address challenges.
- Evaluates project outcomes to identify improvement opportunities, enhance quality, and boost customer satisfaction.
- Drives a culture of continuous improvement by implementing lessons learned and refining project management practices.

Qualifications
Candidates must reside in one of the approved remote worker states: AR, AZ, CO, FL, IL, IN, KY, MI, ND, NE, OH, SC, TN, TX, WI.
- Bachelor’s Degree in related areas or combination of clinical licensure and knowledge acquired through experience; Master's degree preferred.

At least three years of project management experience, demonstrating success in leading complex, cross-functional projects required.

Minimum of two years as a subject matter expert (SME) in home health and/or hospice operations required.
- CAPM certification from PMI required; PMP certification preferred.
- Ability to travel to CommonSpirit Health at Home locations for extended periods will be required.
