Your Key Responsibilities
As a Social Worker Home Health PRN, your primary responsibilities will include:
- Assessing patient needs and developing effective treatment plans
- Providing counseling and therapy services to patients and their families
- Linking patients with community resources and support services
- Collaborating with healthcare teams to ensure comprehensive care
Requirements
To be successful in this role, you will need:
- A Master's degree in Social Work (MSW) or related field
- Licensed Clinical Social Worker (LCSW) certification or licensure in the state where you will be working
- Basic Life Support (BLS) certification from the American Heart Association
- A valid driver's license and reliable transportation
