The Bachelors Social Worker is responsible for performing social work screenings and interventions for hospitalized and emergency department patients in consultation with a Master’s Social Worker (MSW) or a Licensed Clinical Social Worker (LCSW) as needed and collaboratively with the Care Coordination and multidisciplinary healthcare teams. The functions of the Bachelors Social Worker include: patient/family support and referrals, social needs screening, referrals for financial or other identified resource needs, arranging family/patient representative meetings with the healthcare team as needed, assist in post-acute placement of complex discharges and engagement of appropriate agencies or community resources when patient’s social needs are identified. Qualifications

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

Bachelor’s degree from a school of social work accredited by the Council of Social Work EducationPreferred: At least one year of healthcare experience

Bachelor's-level social work licensure, if applicable in the state of practice. For applicants without licensure, licensure must be obtained within 6 months of eligibility, if applicable in state of practice.

Physical Demand Level: Light

This is a full time Social Worker role that requires a MSW.  It does offer full benefits and the schedule is from 8:00 am to 4:30 pm at Mt. St. Francis Nursing Center.  We are looking for applicants that have long-term care experience.

Pay Range

$30.70 - $49.16 / hour

Salary : $31 - $49
