Hello humankindness Dignity Health Medical Group is the employed physician group of Dignity Health Arizona. Dignity Health Medical Group (DHMG) employs approximately 200 providers and 500 support staff that cover a wide variety of specialties. The medical group has had tremendous success over the past few years and now provides more than 73 subspecialty services. The physicians provide clinical services in their areas of specialty and many serve in pivotal academic research and leadership roles.DHMG is also heavily involved in preparing tomorrows healthcare providers. Responsibilities
Coordinates, supervises and implements patient care based on competence, professional expertise, knowledge and evidence-based practice. Practice autonomously, consistent with professional standards, Nurse Practice Act and hospital job description. Input, extract, and evaluate data for meaningful use and patient centered medical home. Responsible for direct patient care, to include rooming, obtaining vitals, patient assessment & education, and discharge planning.
- Assist with Heart Failure Program Development and Coordination
- Aid in Heart Failure Patient Clinic
- Help with Heart Failure Quality Metrics and other Heart Failure related duties
- Acts as a resource and a role model.
- Complies with HIPAA standards.
- Seeks opportunities to increase skills and/or knowledge.
- Participates in department's productivity measures.
- Patient flow: Able to effectively coordinate physician time, patient schedule, and clinical duties of care team.
- Demonstrate initiative and flexibility with day to day activities.
- Demonstrates behavior consistent in promoting excellent patient satisfaction.
- Able to effectively address and resolve patient/customer service concerns; adept at service recovery.
- Identify yourself and your role to the patient and describe pending interaction with patient.
- Provide status updates to patients/providers/team/management as needed.
- Responsive to patient/provider/team/management requests within the department/SJMG/SJHMC guidelines.

Qualifications
MINIMUM
Completion of RN program.
Greater than two years of RN experience.
AZ RN license or RN License from a Compact State required
American Heart Association CPR BLS
Communication, Team building, Clinical competency pursuant to department requirements.
PREFERRED
BSN
Greater than two years of outpatient/ambulatory facility experience.
Bi-lingual, Leadership, Proficient with MS Office programs
Team leader or coordinator
