Job Description
Pharmacy Clinical Specialist in Solid Organ Transplant with Abdominal Organ focus
The Solid Organ Transplant Kidney/Liver Clinical Specialist is responsible for the delivery and documentation of pharmaceutical care provided to patients throughout the medication use process (selecting, prescribing, dispensing, administration and monitoring of medications). The Clinical Specialist provides clinical direction for the pharmacy and acts as a pharmacy liaison. The pharmacist in this position actively participates in inpatient rounding and outpatient clinic services, drug therapy management, and research/ quality improvement activities.
This position would focus on the kidney/liver inpatient and outpatient service with cross coverage of the thoracic lung transplant team if needed.
This position is critical to the success of the Transplant Center at Dignity Health St. Joseph's Hospital and Medical Center and requires the full understanding and active participation in fulfilling the Mission of Dignity Health.
The Solid Organ Transplant Pharmacist:
Provides direct patient care and transplant education
Acts as pharmaceutical consultant to provide drug information to physicians and nurses
Precepts pharmacy practice residents and pharmacy students
Drives pharmacy initiative development and implementation, and provides continued oversight
Acts as a pharmacy representative in clinical, quality, and regulatory activities pertaining to the optimal use of medications
Performs order verification and completes kinetics consults for associated hospital areas
Contributes to P&T Committee activities as appropriate
Actively participates on patient care rounds with providers in a collaborative practice model
Participates in kidney and liver transplant outpatient clinics
Provides comprehensive pharmacotherapy services at the top of her/his license
Qualifications
Requirements:
Graduate degree in pharmacy
Current Arizona pharmacist license in good standing
PGY1 Pharmacy Residency
Preferred Qualifications:
PGY2 Residency in Solid Organ Transplant, Critical Care or Internal Medicine
Two or more years of acute clinical pharmacy experience highly preferred
BPS Board Certification (BCPS or equivalent)
ACLS Certification
Strong clinical pharmacy skills
Experience in precepting both pharmacy residents within an ASHP accredited PGY1 pharmacy residency program and APPE students from ACPE accredited Colleges of Pharmacy
Experience with Cerner Millennium
