Responsibilities
Position Summary:
The Health Coach is an important clinical participant in the Integrated Teamlet Model for practice transformation. They help members navigate, communicate and advocate for their health needs, and assist with health self-management activities. Health Coaches help members develop a self-management plan that is individualized, strength and recovery based.
Qualifications
Minimum Requirements:
- 2-4 years experience with care coordination, case management or discharge planning within a physician practice, medical facility, hospital or health organization.
- Bachelor degree in health care related field with case management level experience.
