Responsibilities : JOB SUMMARY

This job is responsible for collaborating with a Virginia Mason Franciscan Health (VMFH) interdisciplinary team, within scope of licensure or certification, in planning, implementing and documenting care from admission to discharge for patients experiencing a broad range of emotional, psychological, or physical problems directly or indirectly related to substance use. An incumbent participates in departmental activities to promote delivery of quality patient care, effective problem solving and communication, and continued clinical development within scope of licensure.

ESSENTIAL JOB FUNCTIONS :

Performs comprehensive clinical screenings on individuals who present to CHI Franciscan who are at risk for unhealthy substance use along the continuum of mild, moderate and severe substance use disorders.

  • Utilizes validated tools (e.g. Alcohol Use Disorders Identification Test-Consumption (AUDIT-C) Plus 2, etc.) to screen for unhealthy substance use.
  • Identifies appropriate level of care utilizing American Society of Addiction Medicine criteria and knowledge of the full continuum of treatment services available at FHS. Initiates admission process if appropriate.
  • Performs all levels of pre-admission assessment, from inquiry to bio-psychosocial assessment, as well as aspects of the clinical admission process.
  • Works with payor sources to facilitate pre-authorization of admission and treatment.

Develops a treatment plan for each identified problem with appropriate, measurable, and time-linked interventions.

  • Periodically evaluates and revises a treatment plan specific to the age appropriate individual patients problems identified in the Board of Pharmaceutical Specialties (BPS) assessment.
  • Develops diagnostic summaries following a review and evaluation of relevant patient information from all members of the multi-disciplinary team.

  • Initiates, generates and maintains patient records according to established policies, procedures and relevant regulations.
  • Develops and delivers lectures specific to substance use disorder-related issues based on knowledge of learning styles, age, gender, family and cultural issues.

  • Provides therapeutic individual and family sessions.
  • Develops comprehensive discharge plans to include appropriate referrals for continued treatment of identified issues.

  • Assists in the identification and utilization of available support systems and community resources.
  • Acts as a resource for substance use disorder education for staff and contributes to the clinical development of peers, colleagues and others.

    Cultural Sensitivity and Competence : Demonstrates proper use of communication tools / materials for effective communication and understands how the culture(s) of patient populations can affect communication, collaboration and the provision of care, treatment and services. Patient Population Served : Demonstrates knowledge and proper skills associated with the departments defined specific populations served.

    Performs related duties as required.

    Qualifications :

    MINIMUM JOB QUALIFICATIONS :

    Education / Work Experience Requirements

    Education commensurate with licensure / certification, and two years of directly-related work experience with individuals who may have mental health and / or substance use disorders.

    Licensure / Certification

    Hold at least one of the following clinical licensures / certifications as described in the Washington Administrative Code :

  • Substance Use Disorder Professional
  • Mental Health Counselor
  • Marriage and Family Therapist
  • Social Worker
  • Psychologist
  • ARNP
  • Proof of completion of agency-approved SBIRT training is required within three months of date of hire.
  • Current Washington State Drivers License or evidence of equivalent mobility.
