Summary :
The Social Worker will plan, implement and evaluate care plans to ensure proper care within the PACE program. Hours are Monday-Friday, 8a-4p.
Responsibilities :
- Collaborate with the interdisciplinary team to evaluate a patients needs, form a care plan and reevaluate when necessary.
- Facilitate hospital, rehab and nursing home admissions and discharges as needed.
- Arrange and facilitate family meetings when necessary.
- Assist in transitioning patients from home living to long term care when needed.
- Assess psychological needs of patients with the help of behavioral health providers.
