About the Role
The Registered Nurse provides comprehensive nursing care to patients in their homes, as prescribed by physicians and in accordance with applicable laws and regulations. This involves coordinating with other healthcare professionals to develop and implement effective care plans that meet each patient's unique needs.
In addition to delivering direct patient care, the RN will also:
- Collaborate with healthcare teams to ensure seamless continuity of care
- Communicate effectively with patients, families, and caregivers to address concerns and promote understanding
- Maintain accurate records and reports to track patient progress and outcomes
Key Responsibilities
- Assess patients' physical and emotional needs to develop personalized care plans
- Implement care plans through medication administration, wound care, and other treatments
- Monitor patients' conditions and adjust care plans as needed
Requirements
- Current RN licensure in the state of Kentucky
- Current CPR certification
- Current driver's license, vehicle insurance, and access to reliable transportation or public transportation
