Job Description
- As a RN Case Manager, you will be responsible for providing comprehensive care to hospice patients, including initial evaluations, ongoing assessments, and development of individualized plans of care.
Key responsibilities include:
- Making the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral.
- Identifying the patient's/family's physical, psychosocial, and environmental needs and re-assessing as needed, no less than every fifteen (15) days.
- Initiating and coordinating the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition.
