DESCRIPTION OF WORK
As an Aging Care Manager 2, you will provide services from the point of initial contact with older adults, their families, and other professionals through ongoing care management responsibilities. Some specific duties include maintaining a caseload of consumers and providing assistance with protective services, care transitions, and person-centered counseling. This position also involves regular coordination of services with physicians, community agencies, financial institutions, and more. Our ideal candidate must have the ability to manage multiple tasks and demonstrate close attention to detail, which includes completion of paperwork, work assignments, and meeting deadlines. As abilities and understanding of available programs progress, this professional will be assigned increased responsibility for arranging consumer services.
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.
- On-call hours when required.
- Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
QUALIFICATIONS
Minimum Experience and Training Requirements:
- Six months as an Aging Care Manager 1 or a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.

- Hiring preference for this vacancy may be given to candidates who live within Lackawanna, Monroe, Pike, Susquehanna, or Wayne counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
- You must be able to perform essential job functions.

- You must pass a background investigation.
- A conditional offer of employment will require a drug screening.

This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
