Join our team as an Aging Care Manager 2 and find fulfillment in your professional work developing care plans and managing services for older adults, strengthening family life and assisting older individuals in attaining or maintaining independence and self-care with our team! 

As Aging Care Manager you will be responsible for maintaining, creating, and updating ongoing care plans and assessments for consumers who are receiving in home services. Your work is essential to support our goal of providing supportive services while encouraging independence. This will include obtaining information from consumers and their contacts to identify social, economic, emotional, health, or physical problems to help determine which services would be the most helpful for each individual. Work also involves making home visits and phone calls, completing timely assessments and documentation, and working in close cooperation with other community resources to assist customers with utilizing any services needed.


You will have the opportunity to participate in trainings to learn the policies, procedures, rules, and regulations that govern the programs administered by the Monroe County Area Agency on Aging. Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.
  • Travel, overtime, and after hours may be required.
  • Telework: You will not have the option to telework in this position. 
  • Salary: In some cases, the starting salary may be non-negotiable.

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or 
  • Successful completion of the County Social Casework Intern program; or 
  • Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or 
  • An equivalent combination of experience and training.

  • Hiring preference for this vacancy may be given to candidates who live within Monroe County. If no eligible candidates who live within Monroe County apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.


Legal Requirements: 

  • This position falls under the provisions of the Older Adult Protective Services Act.
    • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
