DESCRIPTION OF WORK
As a Health Care Nurse Surveyor, you will be responsible for inspecting home health facilities to evaluate the quality of care and services provided, as well as ensuring adherence to state and federal guidelines. These facilities include, but are not limited to, hospices, end-stage renal disease facilities, rural health clinics, state-licensed home health care agencies, birth centers, and pediatric extended care centers. You will gather and document survey data, apply regulations to survey findings, and prepare detailed survey reports. Additionally, you will conduct complaint and incident investigations, occupancy inspections, and revisits to confirm that the facility has followed their plan of correction to eliminate an identified problem. Your responsibilities will be carried out independently, with a partner, as a member of a team, or as the designated team coordinator during on-site surveys.
Travel throughout the state, including overnight trips and frequent travel within Armstrong, Cameron, Clarion, Clearfield, Elk, Forest, Jefferson, McKean, and Venango counties, is necessary.

Work Schedule and Additional Information:
- Full-time employment, 37.5 hours per week
- Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
- Overtime: As needed
- Salary:In some cases, the starting salary may be non-negotiable.

QUALIFICATIONS
Special Requirements:
- This position requires successful completion of the relevant provider-specific federally mandated Surveyor Minimum Qualifications training and Test (SMQT) provided by the Centers for Medicare and Medicaid Services or an equivalent certifying body for the discipline, within one year of hire.
- T his position requires active authorization to practice as a Registered Nurse in Pennsylvania.
- If you possess an active temporary practice permit, you must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.
- This position requires possession of an active motor vehicle license.
