Job Summary :
As a Medical Care Coach, you will be part of our Complete Care Program. You will play a vital role in our healthcare organization by providing personalized support and guidance to patients to help them navigate the complex healthcare system and improve their overall health and wellness. Commonwealth Pain and Spine is looking for passionate, tech savvy team members to serve our patients who are enrolled in Medicare’s Chronic Care Management Program In this role you will work closely with patients to empower them to make informed decisions about their healthcare, manage chronic conditions, and achieve their health goals. This role requires a deep understanding of medical care, strong phone communication skills, and a compassionate approach to patient support.
This is a Full Time, hourly position
