Job Summary :
The Complete Care Department Supervisor is responsible for overseeing the operations and success of the Complete Care Department, including Chronic Care Management (CCM), Remote Therapeutic Monitoring (RTM) and Behavioral Health Integration (BHI). This role ensures the highest quality of patient care by leading a team of professionals, coordinating services, and maintaining compliance with organizational standards and healthcare regulations. The supervisor plays a vital role in streamlining processes, fostering collaboration, and achieving departmental goals to provide comprehensive, patient-centered care. The Complete Care Department Supervisor is responsible for all aspects of the Complete Care Program as well as training, mentoring, and coaching new and current employees. Responsibilities
Team Leadership and Supervision :Lead, mentor, and manage the Complete Care Department team, including nurses, care coordinators, and administrative staff.
Monitor team performance, provide feedback, and conduct regular performance evaluations.Foster a positive and collaborative work environment to ensure employee engagement and retention.Assist in creating and implementing new processes’ training manuals and guides for new employees.Ensures adherence to the Company mission, vision, values, and patient guarantees.Patient Care Management :Oversee the delivery of Chronic Care Management (CCM) and other coordinated care services to ensure exceptional patient experiences and outcomes.
Ensure patients receive comprehensive care plans, education, and follow-up tailored to their individual needs.Address patient concerns and collaborate with the team to resolve issues promptly.Analyze customer satisfaction and dissatisfaction data to recommend solutions to root cause.Operational Oversight :Manage daily department operations, including staffing, scheduling, and workflow optimization.
Monitor key performance indicators (KPIs) and implement strategies to meet departmental goals.Develop and maintain policies and procedures to ensure compliance with healthcare regulations.Ensuring that Call Center Software and Hardware is functional and working at all times. Troubleshooting technical difficulties and working with IT staff and vendors to ensure downtime and problems are addressed as quickly as possible.Serves as the subject matter expert on everything pertaining to the Complete Care Services Team workflow.Coordination and Collaboration :Serve as the primary liaison between the Complete Care Department and other departments within the organization.
Collaborate with physicians, nurse practitioners, and other healthcare providers to ensure seamless coordination of patient care.Work closely with leadership to align departmental objectives with organizational goals.Quality Assurance and Compliance :Ensure compliance with HIPAA, CMS, and other relevant regulations governing Chronic Care Management (CCM), Remote Therapeutic Monitoring (RTM), and Behavioral Health Integration (BHI).
Implement and monitor quality improvement initiatives to enhance patient outcomes and operational efficiency.Conduct audits and reviews to ensure adherence to policies and standards.Monitor calls to provide feedback regarding telephone etiquette, appropriate information given related to health conditions and follow-up needs addressed.Analyze performance from quality assurance data and communicate trends to Director, Complete Care Services.Data Management and Reporting :Oversee accurate and timely documentation in electronic health records (HER) systems.
Analyze department performance data and prepare reports for leadership review.Use data insights to identify areas for improvement and implement corrective action.All other duties as assigned.Required Skills
Strong knowledge of Chronic Care Management (CCM), Behavioral Health Integration (BHI), and Remote Therapeutic Monitoring (RTM) regulations and best practices.Proficiency in using EHR systems and healthcare software.Excellent organizational, communication, and problem-solving skills.Ability to motivate and lead a diverse team in a fast-paced environment.Exceptional communication and interpersonal skills to interact effectively and tactfully with all levels of employees, management staff, patients, vendors and the general public.Ability to handle multiple simultaneous tasks effectively and efficiently while maintaining a professional, courteous manner.High integrity, including maintenance of confidential information.Exercises professional judgment and demonstrates good problem-resolution skills.Eagerness and ability to work independently as well as part of a team with flexibility and willingness to learn and take initiative on a variety of tasks and projects.Education :
Bachelor’s degree in healthcare administration, nursing, or a related field or minimum of 3–5 years of supervisory or leadership experience in a healthcare setting, preferably in chronic care management or pain management services.
