What are the responsibilities and job description for the Care Coordination Clinical Assistant position at Commonwealth Primary Care?
GENERAL SUMMARY OF DUTIES: Non-patient facing position, assisting the Director of Quality and care coordination team in the management of gaps in care, transitions of care, payer reporting, and other duties that will improve patient health outcomes and reduce cost of care. Quality gaps in care are identified by multiple sources, including but not limited to, insurance portals, insurance payer data reports, and other sources as they are developed.
SUPERVISION RECEIVED: Reports to Director of Quality and Value Based Care.
SUPERVISION EXERCISED: None.
ESSENTIAL FUNCTIONS:
- Data mining charts for appropriate clinical content based on department workflows and protocols.
- Support team as a primary contact for data management as identified with specific insurance carriers and value-based care partners.
- Pull, format and distribute transition of care reports daily from various sources and post to schedules in EHR.
- Assists with management of payer forms and data to include extraction from payer portals, distribution to the practices or care coordination staff, and management of tracking returned forms.
- Occasionally will serve as a point of contact for patients, addressing quality inquiries, scheduling appointments, and providing support in navigating healthcare services
- Attend payer meetings as needed, and serve as a liaison between the payor and care coordination staff
- Other duties may be assigned as the role progresses.
QUALIFICATIONS:
- High school diploma or equivalent required; associate’s degree or higher in healthcare or a related field preferred
- Experience in primary care or outpatient clinic setting preferred.
- Experience as a Medical Assistant (MA), care coordinator, or clinical or clerical assistant in a healthcare setting highly preferred
- Knowledge of electronic medical record reporting, documentation standards, and HIPPA compliance
- Demonstrates professionalism at all times, maintaining appropriate behavior and demeanor in the office setting, and contributing to a positive, respectful, and collaborative work environment
SKILLS & ABILITIES:
- Strong organizational skills and attention to detail
- Excellent communication skills, both verbal and written
- Knowledge of medical terminology and Familiarity with Electronic Health Records (EHR) systems.
- Proficiency in Microsoft Office including Outlook and Excel Required.
- Intermediate level proficiency with Excel spreadsheets required daily to perform work related tasks to include, but not limited to report download, report formatting, filtering and data manipulation, spreadsheet merges and data integration. Advance level proficiency with Excel for tasks to include, but not limited to pivot table construction and use is not required, but is a plus
- Ability to work effectively on a multidisciplinary healthcare team that includes LPNs, RNs, MAs and other non-licensed clinical support staff
- Ability to thrive in a dynamic healthcare office environment, managing competing priorities while remaining adaptable to changes in processes and business needs
ENVIRONMENTAL/WORKING CONDITIONS: This position is based onsite in a healthcare office setting, with occasional visits to patient care locations. Regular office hours, with occasional flexibility based on department needs.
PHYSICAL/MENTAL DEMANDS: Varied activities including sitting, walking, stooping, lifting, bending, reaching. Must be able to use a variety of office equipment including phone, writing instruments, PC, fax/ copier, scanner.
