Key Responsibilities
Patient Intake & Referral Coordination:
- Receive and process home health and hospice referrals from hospitals, physician offices, and other sources.
- Verify patient eligibility, insurance coverage, and obtain necessary pre-authorizations for services.
- Communicate with patients and families to explain services, care plans, and expectations.
- Ensure all required documentation, medical orders, and consents are completed before admission.
- Work closely with nurses, social workers, and physicians to ensure a seamless transition to home-based care.
- Serve as the liaison between referral sources, patients, and clinical teams to expedite the intake process.
- Provide education to providers and hospital discharge planners about home health and hospice services.
- Maintain compliance with Medicare, Medicaid, and private insurance requirements.
- Accurately document patient demographics, clinical information, and referral details in the electronic medical records (EMR) system.
- Ensure all intake processes follow state and federal regulations for home health and hospice care.
- Monitor and follow up on pending referrals to maximize admissions and ensure timely care initiation.
- Assist in quality improvement initiatives to enhance intake efficiency and patient satisfaction.
- Participate in interdisciplinary team meetings to discuss patient needs and care plans.
- Licensed Practical Nurse (LPN) with an active and unencumbered license in [State].
