What are the responsibilities and job description for the Coordinator Waiver Services position at Communicare Inc?
POSITION SUMMARY : Assists with processing and monitoring CMHC, DMS and DBHDID contractual obligations. Serves as a liaison for affiliate Agencies, Programs and Facilities. Coordinates requests for documentation, assessments / re-assessments, participant meetings, and other required services.
QUALIFICATIONS : Bachelor's Degree in human services or related field. One (1) year experience working with individuals with disabilities
SKILL SETS : Good computer skills, superior oral and written communication skills, articulate, trustworthy and decisive. Other skill sets include but are not limited to highly organized, good anticipatory planning, conflict resolution and analytical skills.
PHYSICAL AND ENVIORMENTAL DEMANDS : Visual acuity necessary for updating files and completing documents. Requires frequent stooping, kneeling and crouching to file records. Hearing and talking is necessary to answer phone and talk with employees and other individuals. Work is light with walking, standing and sitting. Manual dexterity is required to perform work processing and other clerical duties.
JOB SPECIFIC FUNCTIONS :
Serves as a liaison for outside case management agencies.
- Comply with all applicable federal and state laws and requirements.
- Track, prepare and submit required documentation and records.
- Reviews and submits incident reports to DBHDID, as applicable.
- Ensures that requested documentation is provided to a case manager within five (5) business days and that receipt of documents from the case manager occur with five (5) business days.
- Provide access to service sites and participant records for the purpose of monitoring and ensuring services, health, safety, and welfare for mutually supported participants.
- Attempt to attend person-centered plan meetings and make positive contributions to team meetings and person-centered planning.
- Ensure person-centered plan documents and modifications are correct and up to date.
- Notify the case manager promptly if corrections or updates are needed.
- Maintain open and positive lines of communication with other providers.
- Conducts and coordinates MPW Assessments / Reassessments.
- Communicates status of services and potential problems to Assistant Director.
- Provides in-services, programs and maintains contacts with community partners at the regional and state level. Conducts training for the agency on a scheduled and as needed basis.
- Participates in agency continuing quality improvement (CQI) activities
- Work closely with the DD Executive Director to maintain current skills and knowledge.
- Performs other duties as assigned.
