What are the responsibilities and job description for the Coordinator - Transitional Age Youth position at Communicare KY?
This position will be responsible for collaborating with the Behavioral Health and Substance Abuse departments to enhance supports that are developmentally appropriate and evidence-based for youth, young adults, and their families. This position will also be responsible for overseeing and coordinating the continuum of supports for the noted population across the region.
