Performs a variety of patient care responsibilities including taking vital signs, recording patient complaints and histories, drawing blood, patient education, and assisting providers with procedures. This position requires compliance with CommuniHealth Services (CHS) written standards, including the Compliance Plan and policies and procedures (including the Code of Conduct).
Responsibilities:
1. Functions within the scope of their approved competency skills checklist.
2. Performs selected nursing and administrative duties, and assists providers as part of the clinical care team.
3. Conducts pre-visit planning by reviewing and updating patient charts prior to appointment with recent test results and correspondence and determines needed services per practice protocol.
4. Confers with provider regarding any incomplete patient tests/consults or other incomplete orders prior to patient visit.
5. Prepares patients for examination and treatment by escorting them from the waiting area to the exam room and taking histories and vital signs.
6. Prepares, cleans and maintains exam and treatment rooms.
7. Gives injections and draws blood; assists with EKGs, Pelvic exams and office procedures as needed; and provides other patient care services as directed by the providers.
8. Administers medication upon provider order and responds to medication requests per practice protocol.
9. Prepares and maintains clinical supplies and equipment.
10. Triages (including telephone triage) and responds to patient communications within practice policy.
11. Coordinates care with disease management or case management programs as needed.
12. Documents, maintains, copies and scans pertinent information in to the patient medical record.
13. Provides patient counseling/teaching and discharge.
14. Verifies and administers immunizations to patients who are not up to date according to the Louisiana Immunization Network System (LINKS) per provider’s order. Correctly adds the immunizations given into LINKS.
15. Completes all required LINKS certifications.
16. Discards all medical waste in accordance with the clinic waste disposal policy and guidelines established by OSHA and EPA.
17. Cleans and runs quality control on equipment as detailed in the clinic policy.
18. Treats staff, providers, visitors and patients with dignity and respect.
19. Completes required CHS training per policy, maintains compliance with and knowledge of all CHS departmental, facility specific and system-wide policy and procedure, attends mandatory departmental meetings and/or training, and follows CHS Mission, Vision and Values in delivery of all services.
20. Attends and participates in continuing educational programs designed to keep you abreast of changes in your profession.
21. Participates in all appropriate meetings within the organization.
22. Maintains a working knowledge of all health information management issues such as HIPAA, Corporate Compliance, and other such healthcare regulations as well as compliance with HRSA regulations.
23. This position requires work beyond normal working hours.
24. Performs other duties as may be assigned.

Education:
1. Certificate or Associates Degree of Licensed Practical Nursing
2. Current LPN license
3. Current CPR certification
4. Pharmacology Certification preferred but not required
5. Phlebotomy Certification preferred but not required
6. EKG Certification preferred but not required
Environmental/Working Conditions:
1. Works in a well-lighted/ventilated office area of the facility.
2. Is subject to frequent interruptions.
3. Is involved with personnel, patients, visitors, government agencies/personnel, etc., under all conditions/circumstances.
4. Is subject to hostile and emotionally upset personnel, patients, visitors, etc.
5. Is willing to work beyond normal working hours: weekends, holidays, and other shifts/positions as necessary.
6. Attends and participates in continuing educational programs designed to keep you abreast of changes in your profession.
7. Is subject to cuts, bruises, falls, burns from equipment, odors, etc., throughout the work day.
8. May be exposed to blood borne pathogens, infectious waste, diseases, conditions, etc., including the potential exposure to the AIDS and Hepatitis B viruses.
Physical Demands:
1. Must be able to handle occasional lifting and pulling.
2. Must be able to move intermittently throughout the workday.
3. Must be able to function independently, have personal integrity, and the ability to work effectively with patients, personnel, and support agencies.
4. Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
