Responsible for providing general and preventative care, conducting check-ups, treating illnesses, ordering diagnostic tests and prescribing medication for children and adults. This position requires compliance with CommuniHealth Services written standards, including the Compliance Plan and policies and procedures (including the Code of Conduct), as well as assistance in the continuous monitoring, reporting and corrective action processes of corporate compliance.
Responsibilities:
1.1. Screen patients to determine the need for medical attention.
2. Review patient records to determine health status.
3. Takes patient history and/or update history.
4. Perform a physical examination, either comprehensive or problem focused.
5. Make decisions regarding data gathering and appropriate management and treatment of patients being seen for the initial evaluation of a problem diagnosed and stabilized condition.
6. Initiate requests for laboratory and diagnostic studies.
7. Identify normal and abnormal findings in history, physical examination and commonly performed laboratory studies.
8. Perform clinical procedures according to collaborative policies, procedures, and protocols.
9. Arrange for or refer patients to needed services that cannot be provided at the clinic.
10. Delegate duties permissible by the Louisiana State Board of Nursing.
11. Keep and maintain (or cause to be kept and maintained) appropriate records relating to all professional services rendered; attending to all billing reports, claims and correspondence required in connection with his services. Such records are sufficient to enable CommuniHealth Services to obtain payment for all services by furnishing appropriate documentation, and to facilitate the delivery of quality patient care.
12. Maintain confidentiality of patient records and information as required by law.
13. Complete medical record documentation timely.
14. Completes required CHS training per policy, maintains compliance with and knowledge of all CHS departmental, facility specific and system-wide policy and procedure, attends mandatory departmental meetings and/or training, and follows CHS Mission, Vision and Values in delivery of all services.
15. Participates in all appropriate meetings within the organization.
16. Maintains a working knowledge of all health information management issues such as HIPAA, Corporate Compliance, and other such healthcare regulations as well as compliance with HRSA regulations.
17. This position requires work beyond normal working hours.
18. Performs other duties as may be assigned.

4. Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objective, policies and procedures, etc., that are necessary for the department.
5. Must have patience, tact, enthusiasm, and a cheerful disposition, as well as be willing to handle persons based on whatever maturity level they are currently functioning.
6. Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.
7. Must be able to work with minimum supervisory guidance and exercise independent judgment; adapt easily to changes in work assignment and environment, is willing to assume additional responsibility and learn new procedures.
Education:
1. Graduate of an accredited nurse practitioner program
2. Must possess a current and valid Louisiana RN and APRN license
3. Must possess a current and valid Federal DEA and LA CDS license (or working towards licensure)
4. Must possess current CPR certification
Environmental/Working Conditions:
1. Works in a well-lighted/ventilated office area of the facility.
2. Is subject to frequent interruptions.
3. Is involved with personnel, patients, visitors, government agencies/personnel, etc., under all conditions/circumstances.
4. Is subject to hostile and emotionally upset personnel, patients, visitors, etc.
5. Is willing to work beyond normal working hours: weekends, holidays, and other shifts/positions as necessary.
6. Attends and participates in continuing educational programs designed to keep you abreast of changes in your profession.
7. Is subject to cuts, bruises, falls, burns from equipment, odors, etc., throughout the work day.
8. May be exposed to blood borne pathogens, infectious waste, diseases, conditions, etc., including the potential exposure to the AIDS and Hepatitis B viruses.
Physical Demands:
1. Must be able to handle occasional lifting and pulling.
2. Must be able to move intermittently throughout the workday.
3. Must be able to function independently, have personal integrity, and the ability to work effectively with patients, personnel, and support agencies.
4. Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
