Founded in 1974, Community Access continues to uphold and operate as a pioneer of supportive housing and social services in NYC for people with mental health concerns. Our programs are spread across 25 locations and include over 1200 units of supportive housing in three boroughs, the Howie the Harp Advocacy Center, Peer-driven Crisis Respite Center, Adult Home Initiative, Assertive Community Treatment (ACT) and the Intensive Mobile Team (IMT).
Department Overview
Community Access provides affordable housing and supportive services to individuals with psychiatric disabilities, histories of homelessness and substance use, families with histories of homelessness, and other low-income community members.
Position Overview
The Per-Diem Housing Counselor has the primary objective to oversee the supervised transitional housing community of residences with mental health concerns and provide support in the areas of, meal preparation and coordination, medication monitoring, house management, ADL training and coaching, and socialization events. In conjunction with the residents, the Per-Diem Housing Counselor will organize socialization events and evening activities and run recreation groups. Per-Diem Housing Counselor’s must provide close observation and keep accurate records of residents taking their medications and take responsibility for prompt follow-up with supervisor and service coordinator for any missing / inaccurate recordings in the medication book.
Qualifications
- Have an understanding, appreciation, and commitment to the philosophy and mission of Community Access required.
- Minimum of a high school diploma or equivalent (GED)
- Previous Customer Service experience and / or residential service experience, preferred
- Must be fingerprinted and cleared by the NYS Justice Center
- Microsoft Office proficient
