Job Description

Job Title: CHR Support Specialist

Reports To: Director of Behavioral Health

FLSA Status: Non-Exempt

Status: Full-time, remote work occasionally

OVERVIEW:

This position provides support to Comprehensive Harm Reduction program which is a grant funded program from Virginia Department of Health. This position provides support, assistance, linkage, referral, and advocacy services to patients who engage with CHR. This position is responsible for data entry, report generation and remittances for grant requirements.

ESSENTIAL FUNCTIONS:

Patient Engagement and case assistance duties:

  • Develops rapport with patients.
  • Assists in the delivery of CHR by completing initial paperwork for entry and connecting patient with CM or PSS.
  • Participates actively as an aide in the delivery of patient services and the coordination of community support services. The duties may include scheduling, making check - in calls with patient’s, and acting as a liaison with primary care and behavioral health staff.

· Triage phone calls to the CHR/BH team for suicide risk and immediate attention

  • When needed, aids newly assigned and high risk CHR patients in scheduling appointments for medical, dental and further BH services

General Administrative Support duties:

  • Works closely with the BH Director and CHR providers to assist with managing the program.
  • Compiles data and creates reports as requested.
  • Schedules patients for appointments.
  • Confirms appointments.
  • Manages the schedule and tracks caseload of CHR providers.
  • Attends staff, health care team, and other agency meetings as required.
  • Coordinates care with substance use treatment providers in the community as directed by DBH.
  • Support BHD in inventory of resources and ordering supplies

Linkage/Referral duties:

  • Educates patients on navigating Community Access Network services and as needed, links patients directly to necessary community resources.
  • Informs patients of available community services and assists with providing them with appropriate information.
  • Ensures adequate resources available in the CHR area for patient access

SKILLS/QUALIFICATIONS:

Required:

· Completion of bachelor’s degree in behavioral sciences, substance use treatment or Humanities Field

· Near completion of bachelor's degree in behavioral science or Humanities field

· Or education equivalent to 3 years working with behavioral health and /or substance use population.

· a valid VA driver’s license and reliable transportation

Preferred

· At least two years of experience working in a clinical setting with persons diagnosed with mental illness and/or substance use and/or individuals who are homeless.

· Sensitivity and experience working with people of diverse racial, ethnic, and cultural backgrounds.

· Familiarity and comfort working with issues related to sexual health and sexual identity, substance abuse, and/or history of trauma

· Knowledge of Integrated Behavioral Health models

· Understanding of Trauma-Informed Care

· Strong interpersonal, written, and oral communication skills; proven ability to communicate with diverse audiences.

· Strong administrative skills and proficiency in Microsoft Office Suite (Word, Excel, Outlook) and ability to learn new electronic health record and practice management software.
