What are the responsibilities and job description for the Support Coordinator- Full Time position at Community Access Unlimited?
We are currently looking for a Full Support Coordinators to cover Northern NJ Counties.
The role of a Support Coordinator is to enable Division Developmental Disabilities (DDD) eligible adults to create stable, effective life-plans. Responsible for reviewing and overseeing monthly follow-along services given to each adult once the NJISP plan is approved by the Support Coordination Agency or the DDD assigned Mentor. EEO
All Full-time positions include a comprehensive benefits package. Two days of paid in-person training at our Main Office in Elizabeth is required at start of position.
Responsibilities
- Assist individuals create a personalized care plan
- Help participants identify and select support options in the community
- Ensures that required documentation is complete and is in compliance with Agency and DDD regulations and standards
- Conduct in-home visits and monthly phone contact
- Reviews and reevaluate service plans and hours of care as needed
- Creates and maintains billing records for all supports provided
- Log all support activity as required by DDD
