Supervised by: Director of Homeless Services

Supervises: No Supervisory Responsibility

Position Type: Full-Time (40 hrs./week)

Salary: 23.00/hr

General Summary:

We are seeking a dedicated and compassionate Adult Youth Case Manager to support young adults (ages 18-24) involved in the newly awarded Youth Homelessness Demonstration Program (YHDP). This program focuses on providing Transitional Housing and Rapid Rehousing services to youth experiencing homelessness. The Case Manager will work directly with youth to assess their needs, develop personalized plans, and connect them to resources that promote housing stability, self-sufficiency, and long-term success.

The ideal candidate will be passionate about ending youth homelessness, committed to empowering young adults, and skilled in navigating the complex needs of youth in crisis. This position will require travel across Alger, Marquette, Baraga, Houghton, and Keweenaw counties to provide in-person support and coordination.

Key Responsibilities:

1. Case Management & Service Coordination

· Actively engage with youth and young adults who are unhoused or at risk of becoming homeless in a variety of community spaces, such as parks, streets, shelters, or public locations across Baraga, Houghton, Keweenaw, Alger and Marquette counties.

· Build trust-based relationships to encourage youth to seek help and accept support for housing and other essential services.

· Develop and implement individualized service plans tailored to each youth’s goals, with a focus on housing stability, employment, education, and life skills.

· Connect clients to supportive services, including emergency housing, rental assistance, education, employment resources, and mental health or substance use services.

· Ensure ongoing monitoring of youth progress and adjust service plans as needed to ensure successful outcomes in the transitional housing and rapid rehousing programs.

· Work closely with landlords, property managers, and housing authorities to assist with housing placements and ensure tenancy stability.

2. Youth Empowerment & Advocacy

· Provide mentorship and emotional support to young adults, helping them overcome barriers to housing stability and independence.

· Assist youth in setting goals and making decisions that support their long-term well-being and independence.

· Empower youth to navigate challenges, manage crises, and develop coping strategies for difficult situations.

3. Crisis Intervention & Safety Planning

· Provide crisis intervention as needed, including conflict resolution, referrals, and short-term solutions.

· Work with clients to develop personalized safety plans and crisis management strategies to ensure stability.

· Respond promptly to emergency situations related to housing instability or personal safety, and connect clients to emergency services when necessary.

4. Program Administration & Documentation:

· Maintain accurate and timely documentation of client interactions, progress, and case notes, following all program requirements.

· Track client data, outcomes, and milestones related to housing stability, program participation, and other key indicators.

· Complete all required reports and documentation in compliance with program guidelines, grant requirements, and funding stipulations.

5. Collaboration & Neworking:

· Build and maintain strong relationships with local service providers, community organizations, housing authorities, and employers to ensure youth have access to necessary resources.

· Participate in regular team meetings, case reviews, and training sessions to share knowledge and stay up to date with best practices for serving youth experiencing homelessness.

Education

Bachelor’s degree in social work, Psychology, Sociology, Human Services preferred or related experience may be substituted for education.

Experience

· At least 1-2 years of experience in case management or providing direct services to youth, especially those experiencing homelessness or housing instability.

· Experience with Transitional Housing, or Rapid Rehousing models is preferred.

· Knowledge of trauma-informed care practices, crisis intervention, and youth development principles.

Skills & Abilities:

· Strong interpersonal and communication skills, with the ability to build rapport and trust with a diverse group of individuals.

· Knowledge of housing resources and diversion programs available for homeless populations in Baraga, Houghton, Keweenaw, Alger and Marquette counties.

· Ability to work independently and as part of a team in a fast-paced environment.

· Strong organizational and time-management skills.

· Compassionate, non-judgmental, and client-centered approach with a commitment to supporting youth in overcoming barriers to housing stability.

· Ability to maintain confidentiality and adhere to ethical and legal standards in all client interactions.

· Proficiency or the ability to learn Microsoft Office and case management software.

Working Conditions:

· The position may require some fieldwork in various outdoor environments.

· This position does require in person home visits to clients in the program.

· Travel across Baraga, Houghton, Keweenaw, Alger and Marquette counties is required, including using a personal vehicle. Mileage reimbursed at .50 cents per mile.

· Must be able to work in a variety of settings, including shelters, community spaces, and public areas.
