Job Title: Bilingual Transitional Age Youth (TAY) Housing Case Manager
Job Summary: The Housing Case Manager (HCM) will provide critical support to transitional aged youth (TAY) in Marin County experiencing homelessness or who are precariously housed. Specifically, the HCM will assist TAY client in navigating services lead to safe and affordable housing. Reporting to the Transitional Aged Youth (TAY) Housing Services Program Manager, the HCM will work in close collaboration with the TAY Peer Support and Outreach Specialists as they connect and build trust with TAY clients. In addition to assessing needs and identifying qualifying services, the HCM will develop customized client plans, facilitate active referrals to services, and close the loop to ensure clients are able to utilize services and access the housing they need.
Primary Responsibilities:

Gather information and conduct client evaluation to develop and implement housing support interventions.

Provide a variety of case management and support services to clients including transportation, placement assistance, advocacy, referrals and assistance with other client support services.

Meet regularly with clients to help facilitate housing application processes.

Participate in regular meetings with TAY Team and other housing case managers to ensure coordination and continuity of care.

Implement consistent communication with agency colleagues to provide timely matching of client need to available resources.

Work collaboratively with social workers, case managers and coordinated entry programs to facilitate successful referrals.

Advocate on behalf of clients as appropriate.

Problem-solve housing issues with clients as appropriate.

Attend all required meetings and trainings.

Perform other related duties as assigned.
Qualifications

Knowledge of housing resources in Marin County, Housing and Urban Development (HUD) vouchers, Supplemental Security Income (SSI) benefits, representative payee, General Assistance, and food stamps.

Experience working with transitional aged youth including high-risk populations and individuals diagnosed with mental illness and/or substance use disorders.

Experience providing outreach services in a community setting, including advocacy, information and referral, case management, and crisis intervention.

A proven track record of working with evidence-based practices.

Familiarity with recovery philosophy and trauma-informed care.

The ability to thrive in a fast-paced, multi-disciplinary environment.

Prior success at establishing and maintaining collaborative relationships with all stakeholders, including families and clients.

Strong communication skills and the ability to interact with a diverse customer base.

The ability to be a team player and work independently.

Empathy, positivity, and patience.

Bilingual proficiency in English and Spanish required – oral and written.

Valid CA Driver License and clean DMV record.
Salary : $32 - $35
