Responsibilities
Team Leadership and Supervision
- Train and supervise CARE Team members, assigning locations, priority populations, and activities in alignment with contract requirements.
- Plan and oversee implementation of all outreach practices and protocols, including distribution of materials, food, hygiene kits, and other needed supplies, and ensuring staff are appropriately equipped for the geographic terrain and client needs.
- Hold weekly staff 1:1s and facilitate monthly team meetings.
- Build a positive and supportive team environment to celebrate successes, trouble-shoot challenges, and model and promote self-care.
- Ensure timely input and completion of client data, case notes, housing plans and other critical data into internal and external databases. Review data regularly in support of staff accountability and client progress.
- With support from Director, ensure compliance with all service and grant-funded outreach contracts, including service delivery, data collections, and reporting.
- Coordainate and collaborate with CAM departments to develop processes and workflows in support of program and client success, including Learning and Evaluation, Operations, Central Kitchen and Finance.
- Support coordination of Point In Time counts, needs assessments, and other population partnership and survey efforts.
- Collaborate with and serve as point-of-contact for Marin County shelters, health and social service providers, local government, and police and emergency services to coordinate outreach efforts and ensure effective service referrals.
- Conduct outreach and perform client screenings and complete individualized care plans as necessary. Coordinate care with assigned case management staff.
- Participate in staff meetings in order to address client needs, program issues, and assist in the integration of services for clientele.
- Assist in the preparation of grants and service contracts.
- Participate in applicable meetings, trainings, conferences, etc. as required both internally and externally.
- Model and nurture trauma-informed and culturally-competent practices, communications and approaches with staff, clients and community.
- Adheres to all client confidentiality requirements & standards.
- Performs other duties as assigned.
Qualifications and Skills
- Bachelor’s degree in social work, psychology, human services, or related field, or equivalent professional experience managing social service programs.
- 4 years of increasing responsibility in homeless services programs, including at least two years of supervisory responsibility; experience working with individuals living with a mental illness and substance use disorder; knowledge of co-occurring disorders and evidence-based treatment approaches.
- Deep knowledge of best practices in outreach and familiarity with case management practices within the unhoused community.
- Proficient in the area of program development and evaluation approaches.
- Working knowledge of Homeless Management Information System or equivalent electronic client records systems.
- Knowledge of state and federal regulations relating to mental health services and substance use disorder treatment; including HIPAA, 42 CFR Part 2, and Medi-Cal insurance.
- Valid California Driver’s License with clean driving record and insurable by Community Action Marin’s automobile insurance policy.
- Deep understanding – and ability to articulate – the role equity, inclusion, and diversity plays in our work.
- Bilingual in English and Spanish preferred.
- Strong communication skills with ability to relate to a diverse population.
