JOB SUMMARY
Position Title: Resource & Family Liaison
Program: Thrive to Five
Classification: Non-Exempt/Full Time
DUTIES AND RESPONSIBITIES
Project Implementation
• Implement recruitment strategies, participate in community events to build a waiting list, complete applications, gather information to select children, inform the Enrollment Supervisor of openings, and collaborate to fill vacancies.
• Work with community partners to communicate about waiting lists, vacancies, and recruitment efforts, ensure timely enrollment, and monitor attendance with teachers and families.
• Monitor child attendance and collaborate with families to enhance attendance, as necessary.
• Provide short-term support, resources, and referrals to individuals to quickly resolve crises, and aid in
navigating obstacles to resources or services of other organizations or businesses, including advocating for clients, assisting with time management, supporting in completion of paperwork, and creatively finding connections for new resources.
• Maintain open communication with your supervisor and serve as a liaison between individuals and other community resources.
• Provide targeted, specialized services focused on special projects’ outcomes.
Data Management
• Update database of current community resources and services.
• Update financial trackers as appropriate for projects following policies.
• Conduct an assessment that matches the person with the right resources at the right time.
• Maintain current and accurate records and case notes of the person or family being served including any goals that the customer has set to obtain; maintain confidentiality.
Child and Family Services
• Complete family assessments to help families set goals, establish plans, and conduct home visits to follow up on these plans and services accessed.
• Collaborate with supervisors and teachers to prioritize caseloads and support families in crisis, documenting follow-up, and results.
• Support families in registering for kindergarten.
• Inform families of scheduled health screenings and ensure attendance, while following health standards and ensuring compliance.
• Collect required health information, collaborate with the health team for family follow-up, and enter health-related information into the database to track trends impacting program effectiveness.
Family Engagement
• Deliver family engagement practices based on funded requirements and encourage family participation in group activities.
• Engage families in center committees to promote family engagement and identify solutions to barriers for attendance.
• Monitor and reflect on the effectiveness of family engagement practices.
• Maintain a list of interested parent volunteers and track their efforts.
Partnerships
• Collaborate with various CAP programs to ensure stability and partnership.
• Collaborate with identified referral partners and external project leads for special projects.
• Participate in inter-disciplinary teams as appropriate with other partners.
• Connect to other community programs to ensure viable partnerships are developed to enhance program delivery and referral process.
• Collaborate with school districts and community partners, attend community meetings, and serve as a communication bridge between schools and families.

EDUCATION/EXPERIENCE REQUIREMENTS
- Bachelor’s degree in social work, human services, family services, counseling, or related field is preferred.
- Associate degree in social work, human services, family services, counseling, or related fields is accepted.
- High School Diploma and within 18 months of hire, at a minimum, a credential or certification in social work, human services, family services, counseling or a related field is acceptable.
