JOB SUMMARY
Position Title : Resource Liaison
Program : RISE
Classification : Non-Exempt / Full Time
DUTIES AND RESPONSIBITIES

Project Implementation
- Provide short-term support, resources, and referrals to individuals to quickly resolve crisis
- Maintain open communication with your supervisor
- Serve as liaison between the individual and other community resources
- Provide targeted, specialized services focused on special projects outcomes
- Aid in navigating any obstacles to resources or services of other organizations or businesses, including, but not limited to, advocating for client, assisting with time management, supporting in completion of paperwork & creatively finding connections for new resources.
Data Management
Update database of current community resources and servicesUpdate financial trackers as appropriate for projects following policiesConduct an assessment that matches the person with the right resources at the right timeMaintain current and accurate records and case notes of the person or family being served including any goals that customer has set to obtain; Maintain confidentialityCoaching Practices & Implementation
Complete work with a family-centered approachIdentify customers who are seeking longer-term support that would benefit from coaching services in collaboration with teamUtilizing Motivational Interviewing techniques, use reflective listening and other self-reflection tools to aid the person or family in reaching greater stabilityParticipate in professional and community development such as Poverty Simulation, Community Navigation, and other trainings as designated.Partnerships
Collaborate with various RISE and CAP programs to ensure stability and partnership.Collaborate with identified referral partners for special projectsParticipate in inter-disciplinary teams as appropriate with other partners and counties.Connect to other community programs to ensure viable partnerships are developed to enhance program delivery and referral process.Innovation and Professional Development
Participate in regular meetings and ongoing professional development in relation to project and organizational goalsParticipate in program evaluation, development and problem solvingImplement best practices for projects; In collaboration with team, identify new best practicesEDUCATION / EXPERIENCE REQUIREMENTS
Associates degree or four-year degree with minimum of three years’ experience or High School Diploma or equivalent minimum of five years’ experience working in Human Services or similar fieldExperience working with low-income families and community services, preferredExperience and education with community health work, preferredProven track record of maintaining a positive work atmosphere by behaving and communicating in a manner that fosters good relations with clients, customers, co-workers and supervisorsAbility to work independentlyAbility to be flexible and problem solve during new initiatives.Ability to treat customers, co-workers and public in a respectful and courteous manner at all timesAbility to develop a sense of community with staff, program participants and volunteersAbility to remain calm and focused on the face of another person’s distressAbility to develop a supportive, empowering culture for diverse members of the communityValid PA driver’s license must possess vehicle and be able to travel to various locations throughout Lancaster County.
